Travel Teams General Information

2007-08 TRAVEL TEAM TRYOUTS: THERE IS A NON-REFUNDABLE TRYOUT FEE OF $50. Registration forms should be filled out on your computer and brought to the rink prior to getting on the ice. Just right-click on the link below and select “Save Target As…” to save the form on your hard drive.  While doing so, substitute your player’s last and first names for “LastName” & “FirstName” in the filename, leaving the underscores in place.  If you saved your data from last year’s form, you can load that data instantly by using the “Load…” button on page 1 and make only the relevant changes (age, birthday, teams, date, etc.). Once you have filled out the forms, please print them out and bring them to the first night of tryouts. If you are having problems with the form, please contact Perry Kotval at 838-4180.  IN ADDITION, YOU MUST BRING CURRENT PROOF OF USA HOCKEY MEMBERSHIP FOR THE 2007-08 SEASON. YOU CANNOT TRY-OUT WITHOUT IT.
